Notice to Class Members: Under the Court‑approved Modification of the Plan of Allocation dated May 19, 2026, all Eligible Class Members are now required to submit a valid Proof of Claim Form in order to participate in the distribution. A copy of the Modification of the Plan of Allocation is available in the Court Documents section of the website.

How to Report a Change of Name or Address

If your address has changed, please send your updated address information.

Marriage:
If your name has changed due to marriage, please send a copy of your marriage certificate.

Divorce:
If your name has changed due to divorce, please send a copy of your divorce decree.

Court Proceedings for Change of Legal Name:
If your name has changed due to court proceedings to legally change your name, please send a copy of the court order of legal name change.
